Job summary

Three Oaks Hospice is seeking a passionate PRN On-Call Registered Nurse (RN) to join our team serving the Athens and surrounding area. You will provide skilled nursing care in patients' homes, coordinate with the care team, families, and referral sources, and help ensure seamless, compassionate hospice services.

The role emphasizes patient-centered care, strong communication, and the ability to support families facing serious illness. CPR certification and an active RN license are required.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state RN license and driver’s license.
• Two years’ RN experience in a clinical setting; hospice or home health preferred.
• Current CPR certification.
• Computer proficiency; Microsoft Office experience preferred.
• Strong interpersonal skills with patients and families.
• Ability to cope with family emotional stress and varying lifestyles.

Responsibilities

• Administer skilled nursing care for clients in their homes.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
• Assist in coordinating patient care across services.
• Maintain detailed documentation of care and plans.

POSITION SUMMARY:

Primary functions are to administer skilled nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing
• Current state license as an RN and current state driver’s license
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred
• Current CPR certification
• Proficient skill using a computer is required; Microsoft office suite experience is preferred
• Strong interpersonal skills and ability to provide effective support to the patient and family
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles
