Job summary

Ilocatum is seeking a compassionate Hospice Nurse (LPN or RN) for a 7on/7off schedule in the East Kansas City Metro region. The role supports patients in their homes or communities after hours, coordinating care with an interdisciplinary team and utilizing community resources to meet patient and family needs.

The ideal candidate holds MO or KS nursing licensure, CPR certification, and 2–5 years of hospice experience, providing compassionate bedside care, symptom management, and caregiver support

Qualifications

• Active nursing license in MO or KS.
• CPR certification.
• 2–5 years of hospice, palliative care, or end-of-life care experience.

Responsibilities

• Provide nursing care and case management for hospice patients in their homes or facilities.
• Assess patient and caregiver needs via phone, home, or facility visits; coordinate care.
• Educate patients and families; manage symptoms and communicate with the interdisciplinary team.
