Job summary

6 Degrees Search is seeking a compassionate Hospice Nurse (LPN or RN) for a 7on/7off, on-call role in the East Kansas City Metro area. You will support patients at home and in facilities, providing symptom management, teaching, and coordination of care after hours.

The role requires MO/KS nursing licenses, CPR, and 2–5 years of hospice or palliative care experience. You’ll collaborate with an interdisciplinary team to meet patient and family needs.

Qualifications

• Active Nursing License in MO and/or KS.
• Current CPR certification.
• 2–5 years in hospice, palliative care, or end-of-life training.
• Knowledge of hospice services and interdisciplinary care.
• Strong organizational and communication skills.
• Analytical problem-solving and attention to detail.

Responsibilities

• Provide in-home and facility visits for assessment, symptom management, teaching and coordination of care.
• Respond to patient/caregiver needs via telephone, home visits or facility visits.
• Collaborate with interdisciplinary team to plan and deliver care.
• Document care activities and maintain accurate records.
