Job summary

Concordia Hospice of Washington is seeking an on-call Licensed Practical Nurse to provide comfort-focused care after hours throughout the South Hills region and Washington County. You will work independently, assess patients, and support families with education and guidance.

The role emphasizes autonomy, compassionate care, and the ability to travel within the service area, with a focus on meaningful, patient-centered outcomes.

Qualifications

• Active Licensed Practical Nurse license required.
• Calm, confident and comfortable working autonomously.
• Compassionate and attentive to patient and family needs.
• Clear and thoughtful communicator.
• Organized, dependable and responsive.
• Willing to travel within assigned territories.

Responsibilities

• Respond to patient needs during evenings, nights and weekends.
• Provide skilled nursing care focused on comfort and symptom management.
• Assess patient conditions and communicate updates to the care team.
• Offer education, reassurance and guidance to patients and families.
• Deliver care with compassion, professionalism and sensitivity.
• Represent Concordia Hospice of Washington with empathy and respect.
