Job summary

Care Initiatives Hospice is seeking a compassionate Registered Nurse (RN) to provide comprehensive hospice care in patients' homes or care facilities across the region. The role emphasizes tailored care with empathy, dignity, and respect, partnering with families and caregivers to meet unique needs.

The RN will perform assessments, develop plans of care, and support interdisciplinary teams while documenting clearly. Travel will be required to serve multiple locations within the region.

Qualifications

• Active Iowa RN license.
• Active CPR certification.
• Hospice or palliative care experience.
• Two years RN/LPN experience preferred.
• Valid driver's license and ability to travel between locations.
• Home internet access.
• Ability to obtain CHPN within 24 months of employment.

Responsibilities

• Use clinical nursing knowledge to deliver high quality patient care in the home or facility.
• Develop and oversee plan of care under physician direction with interdisciplinary involvement.
• Perform initial and comprehensive assessments per policies.
• Conduct ongoing visits and interdisciplinary patient assessments.
• Deliver care with clear, concise clinical documentation.
• Counsel patients, caregivers and families and educate as outlined in the plan of care.

What will you do as a Hospice RN? Here are just a few of the responsibilities:

• Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence.
• Develop and oversee plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.
• Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.
• Continuous visits and interdisciplinary assessment of the patient.
• Deliver quality care and completes clear, concise, and accurate clinical documentation.
• Provide patient, caregiver and family counseling and education as indicated in the plan of care.
• Develop and continually update plan of care that identifies the care and services required to meet patient-specific needs.

What it takes to be an RN with Care Initiatives Hospice:

• Active Iowa RN license.
• Active CPR certification.
• Hospice or palliative care experience.
• Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
• Home access to the internet.
• Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
