Job summary

Care Initiatives Hospice is seeking a compassionate Registered Nurse (RN) to provide end-of-life care across patients’ homes and care settings in Iowa. You will work with families to deliver comprehensive, patient-centered care that respects wishes, values, and dignity.

The RN will assess, plan, and implement care with an interdisciplinary team, ensuring documentation is accurate and timely while supporting patients and families through this profound stage of life.

Qualifications

• Active CPR certification.
• Hospice or palliative care experience.
• Two years of verifiable nursing experience preferred.
• Current driver’s license and ability to travel within region.
• Home internet access to support remote communications.

Responsibilities

• Deliver high quality patient care at the patient’s residence or care setting.
• Develop and oversee plan of care under physician direction with interdisciplinary involvement.
• Perform initial and comprehensive patient assessments per policies.
• Conduct continuous visits and interdisciplinary patient assessments.
• Provide clear, concise clinical documentation.
• Counsel patients, caregivers, and families as outlined in the plan of care.
• Update care plans to meet patient-specific needs.

What will you do as a Hospice RN? Here are just a few of the responsibilities:

• Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence.
• Develop and oversee plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.
• Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.
• Continuous visits and interdisciplinary assessment of the patient.
• Deliver quality care and completes clear, concise, and accurate clinical documentation.
• Provide patient, caregiver and family counseling and education as indicated in the plan of care.
• Develop and continually update plan of care that identifies the care and services required to meet patient-specific needs.

What it takes to be an RN with Care Initiatives Hospice:

• Active Iowa RN license.
• Active CPR certification.
• Hospice or palliative care experience.
• Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
• Home access to the internet.
• Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
