Job summary

Care Initiatives Hospice is seeking an RN Case Manager to provide compassionate, comprehensive care for patients in their home and community settings across Storm Lake, Pocahontas, Sac City and surrounding areas.

The RN Case Manager will assess, plan, and coordinate interdisciplinary care with physicians and families, travel regionally, and ensure documentation and education are delivered with dignity and respect.

Qualifications

• Active Iowa RN license is required.
• CPR certification is required.
• Hospice or palliative care experience is preferred.
• Two years of verifiable RN/LPN work experience preferred.
• Valid driver's license and ability to travel within the region.
• Home access to the internet.
• Ability to obtain CHPN certification within 24 months.

Responsibilities

• Deliver high quality patient care in the patient’s home or other residence.
• Develop and oversee plan of care under physician direction with interdisciplinary involvement.
• Perform initial and comprehensive patient assessments per policies.
• Provide continuous visits and interdisciplinary assessment of patients.
• Ensure accurate clinical documentation and communicate with families.
• Counsel patients, caregivers, and families; educate as indicated in the plan of care.
• Update the plan of care to meet patient-specific needs.

Care Initiatives is looking for a RN to help care for patients in Storm Lake, Pocahontas, Sac City & surrounding areas.

What will you do as a Hospice RN Case Manager? Here are just a few of the responsibilities:

• Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence.
• Develop and oversee plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.
• Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.
• Continuous visits and interdisciplinary assessment of the patient.
• Deliver quality care and completes clear, concise, and accurate clinical documentation.
• Provide patient, caregiver and family counseling and education as indicated in the plan of care.
• Develop and continually update plan of care that identifies the care and services required to meet patient-specific needs.

What it takes to be an RN Case Manager with Care Initiatives Hospice:

• Active Iowa RN license.
• Active CPR certification.
• Hospice or palliative care experience.
• Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
• Home access to the internet.
• Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
