Job summary

Care Initiatives Hospice is seeking a compassionate Registered Nurse (RN) Case Manager to deliver high-quality, patient-centered care in the patient’s residence or other settings such as long-term care communities, assisted living, or hospital.

You will develop and manage comprehensive plans of care, perform assessments, and collaborate with physicians and interdisciplinary teams to meet physical, emotional, and spiritual needs of patients and families in their time of life.

Qualifications

• Active Iowa RN license.
• CPR certification.
• Hospice or palliative care experience preferred.
• Two or more years as RN/LPN preferred.
• Valid driver's license and ability to travel within region.
• Home internet access required.

Responsibilities

• Use nursing knowledge, assessments, and skills to deliver high-quality care at the patient’s residence.
• Develop and oversee plan of care under physician direction with interdisciplinary involvement.
• Perform initial and comprehensive patient assessments per policies.
• Conduct continuous visits and interdisciplinary patient assessments.
• Provide clear, concise clinical documentation of care.
• Counsel patients, caregivers, and families per the plan of care.
• Continuously update care plans to meet patient needs.

What will you do as a Hospice RN Case Manager? Here are just a few of the responsibilities:

• Use clinical nursing knowledge, physical assessment, and procedural skills to deliver high quality patient care in the patient’s place of residence.
• Develop and oversee plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient.
• Perform initial and comprehensive assessments on all patients in accordance with policies and procedures.
• Continuous visits and interdisciplinary assessment of the patient.
• Deliver quality care and completes clear, concise, and accurate clinical documentation.
• Provide patient, caregiver and family counseling and education as indicated in the plan of care.
• Develop and continually update plan of care that identifies the care and services required to meet patient-specific needs.

What it takes to be an RN Case Manager with Care Initiatives Hospice:

• Active Iowa RN license.
• Active CPR certification.
• Hospice or palliative care experience.
• Two (2) years of verifiable experience working as a nurse (RN/LPN), preferred.
• Current driver's license, automobile liability insurance, and the ability to travel between multiple locations within your region.
• Home access to the internet.
• Ability to obtain Hospice and Palliative Registered Nurse Certification (CHPN) within 24 months of employment.
