Job summary

MyMichigan Health is seeking a Staff Registered Nurse to deliver professional nursing care for hospice patients. You will assess, plan, and implement nursing actions within agency policies and physician orders, supporting patient independence and comfort.

Responsibilities include coordinating care with LPNs and Home Health Aides, adapting plans of care, and ensuring accurate documentation. Requires Michigan RN licensure, valid driver's license, and ability to work flexible shifts including

Qualifications

• One year of direct patient care nursing experience; med/surg preferred.
• Active RN licensure in Michigan.
• Access to reliable transportation and ability to work weekends/holidays per policy.
• Ability to communicate effectively, both verbally and in writing.
• Successful completion of Agency orientation and competency programs.
• Valid driver's license and auto insurance.

Responsibilities

• Regularly assesses patient needs and provides skilled nursing services as prescribed by the physician-ordered plan of care.
• Initiates plan of care for assigned patients and revises as necessary.
• Provides nursing care or arranges services through specialized staff during visits.
• Coordinates patient care by evaluating outcomes and updating the care plan as needed.
• Coordinates care provided by LPNs and Home Health Aides and supervises LPNs as needed.
• Notifies physician of changes in patient condition and obtains new orders.
