Job summary

Care Initiatives Hospice is seeking a compassionate Nurse Practitioner to deliver comprehensive hospice care across settings, including home, long-term care, assisted living, and hospital. You will meet patients and families where they are comfortable, coordinating with caregivers for holistic care during life-limiting illness.

The ARNP will provide care with empathy and dignity, traveling within the region and collaborating with the care team to ensure seamless, patient-centered service.

Qualifications

• Current ARNP license and ability to activate in the state where you will be working.
• Active CPR certification.
• Hospice or palliative care experience.
• Two years of verifiable nursing experience (ARNP/RN/LPN); hospice experience preferred.
• Valid driver’s license and ability to travel between multiple locations within your region.
• Home access to the internet.

Responsibilities

• Provide comprehensive hospice care addressing physical, emotional, and spiritual needs.
• Meet patients and families where they are most comfortable (home, long-term care, assisted living, or hospital).
• Collaborate with family members and caregivers as part of the care team.
• Travel between multiple locations within your region to provide care.

What it takes to be a Nurse Practitioner with Care Initiatives Hospice:
