Job summary
VITAS Healthcare in Bakersfield, CA seeks a Nurse Practitioner to provide hospice care as part of an Interdisciplinary Team. You'll assess patients, formulate care plans, prescribe or recommend treatments, and monitor progress with the physician and health professionals.
This part-time, day-shift role requires at least 2 years of hospice or home health experience, NP certification and state licensure. You will collaborate with hospice staff and volunteers to deliver compassionate, high-quality
Qualifications
- Minimum 2 years experience in hospice, home health, or community health in the past 5 years.
- Able to assist in lifting and transferring patients as needed.
- Lift objects >20 pounds.
- Specific vision abilities: close, distance, focus.
- Stand/walk >1 hour daily; talk/hear/sit/kneel as needed.
Responsibilities
- Record findings and formulate care plans.
- Discuss cases with physician and other health professionals.
- Prescribe or recommend drugs or treatments.
- Monitor patient progress and report to supervising physician.
- Collaborate with hospice staff and volunteers as part of an IDT.
