Job summary

Vitas Healthcare Corporation in Illinois seeks a Nurse Practitioner for hospice/home health. You will record physical findings, discuss cases with physicians, and help prepare comprehensive patient care plans.

You will prescribe or recommend drugs or other treatments, monitor patient progress, and report findings to supervising physicians. Requires completion of an accredited NP program and state licensure; at least two years of hospice/home health experience is preferred.
