Job summary

Central Coast VNA & Hospice (VNA) seeks a Part-Time Nurse Practitioner to deliver clinical care in home settings across the Central Coast. You will collaborate with physicians and the multidisciplinary team, oversee individualized plans of care, and conduct face-to-face home visits, documentation, and follow-up care.

Requirements include a California Furnishing license and DEA certificate, a Master’s in nursing (or equivalent), CPR, and strong pain and symptom management knowledge.

Qualifications

• California Furnishing license and DEA certificate.
• Master’s degree in nursing or equivalent advanced practice certification.
• CPR certification.
• Strong knowledge of pain and symptom management principles.
• Excellent interpersonal, communication, and clinical consultation skills.

Responsibilities

• Coordinate patient care and oversee individualized plans of care.
• Conduct face-to-face home visits and provide follow-up care.
• Provide telephone and on-site clinical consultation to staff as assigned.
• Complete clinician documentation and ensure thorough charting.
