Job summary

Northern Arizona Hospice in Cottonwood, AZ, part of the LHC Group within the Optum family, seeks a Hospice Nurse Practitioner to deliver palliative and supportive care to hospice patients as requested by the referring physician.

You will lead face-to-face visits, coordinate with the Hospice Medical Director, and collaborate with a multidisciplinary team to ensure comprehensive patient care and family support. A compassionate, patient-centered approach is essential.

Qualifications

• Active state nursing license (unrestricted)
• Current certification as a clinical Nurse Specialist or Nurse Practitioner
• Current CPR certification
• 5+ years of nursing experience
• Valid driver's license and access to reliable transportation

Responsibilities

• Make face-to-face encounter visits within prescribed time limits to qualifying Hospice patients and report findings to the Hospice Medical Director
• Consult with the Hospice Medical Director and inform the referring physician of recommended services
• Coordinate and document plan of care, goals, interventions and patient/family response
• Instruct and supervise patients/families in self-care techniques when appropriate
• Maintain accurate clinical notes and report patient condition and needs to management and Hospice Medical Director
