Job summary

UnitedHealth Group, through Northern Arizona Hospice of Cottonwood, AZ, is seeking a Hospice Nurse Practitioner to provide consultation in palliative and supportive care to hospice patients as referred by physicians.

Primary responsibilities include face-to-face visits, coordinating care with the Hospice Medical Director and interdisciplinary team, guiding families, and maintaining thorough clinical documentation.

Qualifications

• Active unrestricted nursing license in the state of Arizona.
• Current CPR certification.
• 5+ years of nursing experience.

Responsibilities

• Visit hospice patients face-to-face within prescribed time limits and report findings to the Hospice Medical Director.
• Consult with the Hospice Medical Director and coordinate with primary physicians and other clinicians.
• Initiate and coordinate plans of care; document goals, interventions and family response.
• Instruct and supervise patient/family in self-care techniques when appropriate.
• Maintain accurate clinical notes and update Administrator and Hospice Medical Director on patient status.
