Job summary

Silver-Spring Home Healthcare Services Inc seeks a Nurse Practitioner to provide comprehensive hospice services under physician supervision. The NP evaluates health history, diagnoses conditions, and directs care plans in patients' homes and communities.

The role requires a Masters in nursing, NP certification (Texas RN license noted; FNP/ANP/GNP preferred), and BLS within 3 months. DEA registration eligibility and hospice experience are also valued.

Qualifications

Masters prepared graduate of an accredited school of nursing and nurse practitioner program.
Current Texas RN license and NP certification; FNP/ANP/GNP preferred.
BLS for Health Care Providers required within 3 months of start date.

Responsibilities

Provide hospice care through evaluation, health history, diagnosis and plan of care.
Prescribe medications and treatments; coordinate with patients, families and other practitioners.
Visit patients and perform physical and face-to-face assessments; evaluate hospice eligibility with the interdisciplinary team.
