Qualifications

• Licensed as Nurse Practitioner in Indiana.
• Experience in hospice or home-based care preferred.
• Ability to assess and plan for terminally ill patients.
• Strong HIPAA compliance and patient confidentiality.

Responsibilities

• Care for terminally ill patients at home.
• Assess and reassess nursing needs.
• Perform face-to-face visits timely.
• Provide dietary counseling.
• Deliver hospice nursing services and procedures.
• Coordinate care with IDG and team.
• Maintain accurate clinical records within 48 hours.
