Haven Hospice in Gainesville, FL is seeking a dedicated Hospice RN Case Manager to provide patient-centered care at a critical time. The role emphasizes independent work with strong team support and a focus on quality documentation and compassionate care for patients and families.

The ideal candidate has nursing licensure in Florida, 1 year of nursing experience (3+ years preferred), and knowledge of hospice care principles.

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in state of operation.
• One year nursing experience required, 3+ years preferred.
• Experience in hospice or a similar setting preferred.
• Knowledge of the hospice philosophy of care.
• Commitment to clinical and documentation excellence.
• Must be a licensed driver with a vehicle in good working order.

Responsibilities

• Assess, monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans that align with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.
