Job summary

ExcelaCare Home Health and Hospice is seeking a Nurse Practitioner to join its hospice care team. The NP will assess patients, develop care plans, manage symptoms, and support families through end-of-life care, collaborating with physicians, nurses, social workers, and chaplains to ensure high-quality care.

Successful candidates hold an MSN or DNP, have hospice/palliative experience, and are licensed to practice in the state; they should be organized, compassionate, able to work independently,

Qualifications

• Education: MSN or DNP required.
• Experience: 3 years hospice/palliative care preferred.
• License: valid driver's license and current auto liability insurance; licensed to practice in the state.

Responsibilities

• Conduct comprehensive patient assessments and develop care plans.
• Manage pain and other symptoms per regulations.
• Educate patients and families on disease processes and hospice philosophy.
• Collaborate with interdisciplinary team to coordinate care.
• Maintain accurate clinical documentation and participate in QAPI.
