Job summary

Angels Care Hospice is seeking a Nurse Practitioner to join our hospice team. The NP will visit patients, perform physical assessments, and evaluate eligibility for hospice care in collaboration with the interdisciplinary team.

Under supervision of a physician, the NP will provide medical care and palliative management for terminal illnesses, ensuring adherence to applicable nursing practices and standards of care.

Qualifications

• Active NP license in Kansas (inferred from location)
• Experience in hospice care and palliative management (inferred)
• Strong communication with interdisciplinary teams (inferred)

Responsibilities

• Provides medical care and manages terminal illness in accordance with the state Nurse Practice Act.
• Completes history and physical examinations, evaluating medical condition and health history.
• manages health conditions and terminal illnesses and completes the hospice face-to-face visit requirements.
• Provides clinical information to the hospice medical director for eligibility interpretation for hospice recertification.
• Makes recommendations to the hospice medical director regarding patient eligibility for certification and recertification.
• Reviews clinical information, coordinates and oversees patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners.
