Job summary

Well Care Health is seeking a Hospice Nurse Practitioner to provide advanced clinical assessment, symptom management, and palliative care to hospice patients. You will work with the Hospice Medical Director and interdisciplinary group to determine eligibility and update care plans.

You will educate and mentor clinical staff, participate in after-hours on-call coverage, and ensure accurate documentation per Medicare and state requirements.

Qualifications

• Graduate of NLN-accredited school of nursing and licensed as a registered nurse.
• Current RN license in NC and CPR certification.
• Minimum of one year of clinical hospice experience preferred.
• Strong interpersonal skills and ability to work in an interdisciplinary team.
• Proficiency with Word, email, and EMR documentation.

Responsibilities

• Provide advanced clinical assessment and symptom management for hospice patients.
• Conduct Face-to-Face encounters for eligibility and recertification as required.
• Collaborate with the Hospice Medical Director and IDG on care plans.
• Educate and mentor clinical staff in hospice and palliative care practices.
• Participate in after-hours, weekend, and holiday on-call coverage.
• Maintain accurate clinical documentation in compliance with Medicare/state requirements.
