Job summary

Well Care Hospice, Inc. seeks a Nurse Leader to oversee daily hospice care operations, supervise the interdisciplinary team, and ensure admissions comply with regulations. The role emphasizes effective scheduling, policy adherence, and maintaining confidentiality.

You will work with RN Case Managers, Social Workers, Chaplains, and other clinicians, driving high-quality patient care, staff development, and budget control while upholding the agency's guiding principles.

Qualifications

• Registered nurse with current license in the state.
• NLN-accredited nursing program graduate.
• Bachelor's degree preferred for leadership role.

Responsibilities

• Supervise the hospice care team and serve as a resource to interdisciplinary staff.
• Ensure admissions and care activities comply with state, federal, and accreditation standards.
• Maintain confidentiality and promote Well Care Health guiding principles.
• Lead scheduling, performance improvement efforts, and vendor relationships.
• Participate in IDG meetings and develop plans of care with the team.
