Job summary

Day City Hospice in Dayton, OH is seeking an experienced Licensed Practical Nurse (LPN) to join a patient‑centered hospice team. You will provide direct care, perform assessments, and collaborate with an interdisciplinary team to deliver comfort-focused care.

Three 12-hour shifts per week with every other weekend, plus opportunities for professional growth. Valid OH LPN license and BLS required, hospice/home health experience preferred.

Qualifications

Graduate of an accredited Practical/Vocational Nursing program.
Active Ohio LPN/LVN license.
Minimum 3 years nursing experience; hospice/home health preferred.

Responsibilities

Provide direct patient care per State Nurse Practice Act and hospice standards.
Conduct comprehensive nursing assessments and implement plans of care.
Deliver skilled nursing services focused on comfort, dignity, and quality of life.
Administer and monitor pain and symptom management interventions.
Educate patients and families on disease progression, care plans, and symptom control.
Document care timely in EMR and ensure accuracy.
Collaborate with interdisciplinary team for coordinated care.
Adjust care plans based on patient responses.

Schedule & Work-Life Balance

Key Responsibilities

Provide direct patient care in accordance with the State Nurse Practice Act and hospice standards
Conduct comprehensive nursing assessments and implement individualized plans of care
Deliver skilled nursing services focused on comfort, dignity, and quality of life
Administer and monitor pain and symptom management interventions
Educate patients and families on disease progression, care plans, and symptom control
Document patient care accurately and timely within electronic medical records (EMR)
Collaborate with the interdisciplinary team to ensure coordinated, holistic care
Evaluate patient responses and adjust care plans as needed

Required Qualifications

Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program
Current LPN/LVN license in the state of Ohio
Minimum 3 years of nursing experience (hospice, home health, med-surg, or community health preferred)
Current BLS certification
Strong clinical, communication, and organizational skills
Valid driver’s license, reliable transportation, and proof of insurance
Understanding of hospice philosophy and end-of-life care
