Job summary

Day City Hospice in Dayton, Ohio is seeking a Licensed Practical Nurse (LPN) to provide direct patient care in hospice settings across Dayton and surrounding areas.

This role offers three 12-hour shifts per week, every other weekend, with four days off, a collaborative team, and comprehensive benefits including medical/dental/vision, 401(k), PTO, and tuition reimbursement.

Qualifications

• Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program.
• Current LPN/LVN license in Ohio.
• Minimum 3 years of nursing experience (hospice/home health/med-surg/community health).
• Current BLS certification.
• Strong clinical, communication, and organizational skills.
• Valid driver’s license and reliable transportation.
• Understanding of hospice philosophy and end-of-life care.

Responsibilities

• Provide direct patient care in accordance with the State Nurse Practice Act and hospice standards.
• Conduct comprehensive nursing assessments and implement individualized plans of care.
• Deliver skilled nursing services focused on comfort, dignity, and quality of life.
• Administer and monitor pain and symptom management interventions.
• Educate patients and families on disease progression, care plans, and symptom control.
• Document patient care accurately and timely in EMR; collaborate with interdisciplinary team.

Key Responsibilities

• Evaluate patient responses and adjust care plans as needed

Required Qualifications

• Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program
• Current LPN/LVN license in the state of Ohio
• Minimum 3 years of nursing experience (hospice, home health, med-surg, or community health preferred)
• Current BLS certification
• Strong clinical, communication, and organizational skills
• Valid driver’s license, reliable transportation, and proof of insurance
• Understanding of hospice philosophy and end-of-life care

Preferred Qualifications

• Experience in hospice or palliative care
• Familiarity with home-based care and EMR systems
