Job summary

Day City Hospice in Dayton, OH is seeking a Licensed Practical Nurse (LPN) to join our hospice team delivering compassionate, patient-centered end-of-life care. This role features a stable schedule, with three 12-hour shifts per week and every-other-weekend rotation, supporting work-life balance.

You will perform direct patient care, nursing assessments, pain and symptom control, and collaborate with an interdisciplinary team to ensure comfort and quality of life for patients and families.

Qualifications

• Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program.
• Current LPN/LVN license in Ohio.
• Minimum 3 years of nursing experience (hospice/home health/med-surg/community health preferred).
• Current BLS certification.
• Strong clinical, communication, and organizational skills.
• Valid driver’s license, reliable transportation, and proof of insurance.
• Understanding of hospice philosophy and end-of-life care.

Responsibilities

• Provide direct patient care in accordance with the State Nurse Practice Act and hospice standards.
• Conduct comprehensive nursing assessments and implement individualized plans of care.
• Deliver skilled nursing services focused on comfort, dignity, and quality of life.
• Administer and monitor pain and symptom management interventions.
• Educate patients and families on disease progression, care plans, and symptom control.
• Document patient care accurately and timely within electronic medical records (EMR).
• Collaborate with the interdisciplinary team to ensure coordinated, holistic care.
• Evaluate patient responses and adjust care plans as needed.
