Job summary

Day City Hospice in Dayton, OH is seeking an Licensed Practical Nurse (LPN) to join our hospice team. You will provide direct patient care in accordance with state nurse practice acts and hospice standards, conduct comprehensive assessments, and assist in care planning to enhance comfort and quality of life.

The role offers a three 12-hour shift schedule, every other weekend, and four days off weekly to support work-life balance.

Qualifications

Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program.
Current LPN/LVN license in Ohio.
Minimum 3 years of nursing experience; hospice/home health preferred.
Current BLS certification.
Strong clinical, communication, and organizational skills.
Valid driver’s license and reliable transportation.

Responsibilities

Conduct comprehensive nursing assessments and implement care plans.

Schedule & Work-Life Balance

Three 12-hour shifts per week

Key Responsibilities

Provide direct patient care in accordance with the State Nurse Practice Act and hospice standards
Conduct comprehensive nursing assessments and implement individualized plans of care
Deliver skilled nursing services focused on comfort, dignity, and quality of life
Administer and monitor pain and symptom management interventions
Educate patients and families on disease progression, care plans, and symptom control
Document patient care accurately and timely within electronic medical records (EMR)
Collaborate with the interdisciplinary team to ensure coordinated, holistic care
Evaluate patient responses and adjust care plans as needed

Required Qualifications

Graduate of an accredited Practical/Vocational Nursing (LPN/LVN) program
Current LPN/LVN license in the state of Ohio
Minimum 3 years of nursing experience (hospice, home health, med-surg, or community health preferred)
Current BLS certification
Strong clinical, communication, and organizational skills
Valid driver’s license, reliable transportation, and proof of insurance
Understanding of hospice philosophy and end-of-life care
