Job summary

Bristol Hospice in Merced, CA is seeking a Clinical Liaison to maintain positive relationships with customers and referral sources within the community. This full-time, day-shift role offers a salary range of $110,000 to $130,000 per year.

Bristol Hospice is a nationwide leader in compassionate, family-centered hospice care. The role involves education, intake coordination, and collaboration with hospitals, physicians, and care facilities to ensure high-quality patient experiences and outcomes.

Qualifications

• Active California Registered Nurse License.
• CPR certification and valid driver’s license.
• Minimum of one year experience as a Registered Nurse.
• Must be flexible in work hours.
• Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs.

Responsibilities

• Ensure the educational needs of the community related to hospice care and services are assessed and met on an ongoing basis.
• Establish and monitor community, customer, and payer and patient perceptions of Bristol Hospice as a high-quality provider of hospice services in the community.
• Receive hospice referrals. Conference with clinical supervisor and/or Director of Patient Care Services to collaborate and schedule intake activity.
• Participate in bereavement activities as assigned.
• Participate in social work activities as assigned.
• Serve as a liaison between facility, physicians, hospitals, skilled nursing facilities, and community and hospice program with coordination of referrals/admission process and services.
