Job summary

Bristol Hospice in Lakewood, CA is seeking a Clinical Liaison to nurture relationships with referral sources and the community, ensuring timely intake and high-quality hospice education.

The role requires a social work degree or LVN/LPN licensure with active state credentials, CPR, and a valid driver's license. Expect engagement across facilities, physicians, and care teams in a fast-paced environment.

Qualifications

• Graduate of an accredited school of social work or LVN/LPN with valid state licensure.
• Current LCSW or LPN/LVN licensure in State, CPR certification and valid driver's license.
• Minimum of one year of professional social work experience in the last three years.

Responsibilities

• Assess and meet community educational needs related to hospice care.
• Conduct community education needs assessments and develop plans.
• Maintain knowledge of educational resources and share information with staff.
• Receive hospice referrals and coordinate intake with the Director of Patient Services.
• Participate in bereavement and social work activities as assigned.
• Serve as liaison between facilities, physicians, hospitals, and hospice programs.
• Respond promptly to referrals and inquiries; assist in planning education programs.
• Participate in QAPI activities and contribute to care planning with the IDT.

Qualifications

An Average Day:

(Includes, but not limited to)

• Ensure the educational needs of the community related to hospice care and services are assessed and met on an ongoing basis.
• Continuously conduct community education needs assessments and develop an associated plan designed to meet community services as determined.
• Establish and monitor community, customer, and payer and patient perceptions of Bristol Hospice as a high-quality provider of hospice services in the community.
• Maintain comprehensive working knowledge in the field of community educational resources and share information with appropriate organizational personnel.
• Receive hospice referrals. Conference with clinical supervisor and/or Director of Patient Care Services to collaborate and schedule intake activity.
• Follow intake referral processes.
• Participate in bereavement activities as assigned.
• Participate in social work activities as assigned.
• Serve as a liaison between facility, physicians, hospitals, skilled nursing facilities, and community and hospice program with coordination of referrals/admission process and services.
• Promptly respond to customer and referral source requests and concerns.
• Assist Executive Director and Director of Patient Services regarding the planning, implementation, and evaluation of in-service and continuing education programs.
• Actively participate in the Quality Assurance Performance Improvement (QAPI) process as directed by the Executive Director and/or Director of Patient Services.
• Complete initial social work assessment of patient and family.
• Complete education on hospice service and philosophy.
• Complete consent forms as assigned and indicated.
• Submit associated documentation in accordance with administrative processes.
• Participate in IDT meetings and work with the IDT to determine the patient/family plan of care as needed.
• Provide patient/family education related to hospice philosophy and care.
• Comply with accepted professional standards of practice and principles.

Requirements

• Graduate of an accredited school of social work or a Licensed Vocational Nurse/Licensed Practical Nurse.
• Social Worker in this role shall have a minimum of one year experience as a professional social worker within the last three years or have a Licensed Social Work Degree in social work from an accredited program. Licensed Vocational Nurse/Licensed Practical Nurse in this role shall have a minimum of one year experience as an LVN/LPN within the last three years.
• Current LCSW or LPN/LVN licensure in State, CPR certification and valid driver’s license.
• Must be flexible in work hours
• Must possess a willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs
