Bristol Hospice-CALIFORNIA LLC is seeking a Clinical Liaison in Merced, CA. This full-time role focuses on maintaining positive relationships with customers and referral sources within the community while coordinating hospice referrals and education efforts.

Ideal candidates are licensed RNs in California with at least one year of RN experience, CPR certification, and a willingness to work flexible hours. The organization emphasizes compassionate, high-quality hospice care.

Qualifications

• Active California RN License is required.
• Minimum of one year experience as a Registered Nurse.
• CPR certification and valid driver’s license.
• Must be flexible in work hours.
• Willingness to maintain knowledge of information systems and software.

Responsibilities

• Ensure educational needs of the community related to hospice care are assessed and met.
• Conduct community education assessments and develop plans to meet community services.
• Maintain perceptions of Bristol Hospice as a high-quality provider.
• Receive hospice referrals and coordinate intake with supervisors.
• Follow intake referral processes and participate in bereavement activities.
• Serve as liaison between facilities, physicians, hospitals, and hospice programs, coordinating referrals.
