Job summary

Bristol Hospice – Walnut Creek, CA is seeking a Community Liaison to build relationships with healthcare professionals, community organizations, and local stakeholders, raising awareness of our hospice services.

This full-time, day-shift role requires an active LVN license in California, 1–2 years hospice sales experience, and strong communication skills. You will coordinate referrals, support intake, and educate patients and families about hospice philosophy and care.

Qualifications

• Active LVN license in California.
• 1–2 years hospice sales experience preferred.
• Excellent verbal and written communication skills.
• Ability to walk/stand for long periods.
• Willingness to maintain systems knowledge.

Responsibilities

• Educate community about hospice care and Bristol services.
• Audit charts to ensure compliance.
• Coordinate referrals and admissions with clinicians.
• Respond promptly to referrals and customer inquiries.
• Follow intake processes and documentation.
