Your Health is seeking a Registered Nurse to serve patients across the Aiken, SC area as a Nurse Case Manager. The role involves visiting patients at homes and facilities, coordinating with providers, and initiating telemedicine visits.

Daily travel is required, with on-call rotations for nights and weekends. The position is full-time, with a comprehensive benefits package and opportunities for growth within a physician-led group serving SC and GA communities.

Qualifikationen

• Must be a Registered Nurse with a license in good standing.
• Minimum of three (3) years clinical experience preferred.
• Experience in community settings is preferred.
• Proven ability to communicate and collaborate with care teams, patients, and caregivers.
• Strong written and verbal communication skills; basic computer knowledge.

Aufgaben

• Facilitate receiving and review of medical records from patients’ providers.
• Enroll patients in Care Management and initiate a Plan of Care when eligible.
• Coordinate care with the patient’s health care team and providers.
• Conduct home visits and telemedicine visits as needed.
• Document Care Management activities and patient progress.
