Job summary

Your Health is seeking a Registered Nurse to join our Nurse Case Manager team serving the Augusta area. The nurse will visit patients at home and in facilities, coordinate with primary providers, initiate telemedicine visits, and ensure care plans comply with laws and policies.

Daily travel is required; this is a full-time, salary-based position with 12-hour shifts. We serve South Carolina and Georgia with primary care, specialty services, and pharmacy support.

Qualifications

• Must be a Registered nurse with license in good standing.
• Minimum of three years clinical experience preferred.
• Experience in community settings preferred.
• Proven ability to communicate and collaborate with interdisciplinary care teams.
• Strong written and verbal skills.
• Ability to manage multiple responsibilities and exercise sound judgment.
• Must be able to travel to patients' homes and facilities.

Responsibilities

• Facilitate obtaining medical records from patients' providers and specialists.
• Review medical records and complete consents.
• Enroll patients in Care Management if eligible.
• Initiate Care Management Plans of Care where applicable.
• Document Care Management activities and coordinate with the care team.
• Coordinate visits with providers and telehealth when needed.
• Evaluate for home health, hospice, RPM devices, or other supports.
• Reconcile medications and provide post-discharge education.
