Job summary

Hospice of Dubuque seeks a Hospice Nurse Case Manager to oversee nursing services for assigned patients, collaborating with the Interdisciplinary Team and admitting patients to the program as needed.

You will visit patients at home, assess needs, develop and implement plans of care, provide patient and family education, participate in on-call coverage 24/7, and ensure HIPAA compliance.

Qualifications

• Graduate of a professional nursing program; RN license required.
• BSN preferred.
• Must have current nursing license for state served.

Responsibilities

• Serves as a member of the Interdisciplinary Team.
• Oversees nursing services for assigned patients.
• Visits patients in their home to provide nursing care.
• Assesses patient/family needs for the care plan.
• Conducts admission assessment and initial nursing care plan.
• Implements and evaluates the plan of care.
• Participates in 24/7 on-call access for patients.
• Instructs patients and families in care/services.
• Provides counseling and facilitates communication among family and providers.
• Maintains HIPAA/privacy compliance.
