Job summary

Hospice of Dubuque is seeking a dedicated Hospice Nurse Case Manager to lead nursing care for assigned patients in home settings. You will supervise nursing services, perform initial and ongoing assessments, develop and update care plans, and collaborate with the Interdisciplinary Team to ensure compassionate, high-quality end-of-life care.

Qualifications

• Must have current nursing license for state(s) served.
• BSN preferred.
• One or more year nursing experience preferred.

Responsibilities

• Visit patients in their home to provide nursing care and interventions.
• Assess patient/family needs for development of initial and comprehensive plans of care.
• Perform admission process including initial assessment and establishing coordinated plan of care with Hospice of Dubuque.
• Implement the plan of care, provide direct nursing services and evaluate interventions.
• Participate in on-call to provide 24/7 nursing access.
