Job summary

CommonSpirit Health at Home is seeking a Nurse Practitioner for PRN Hospice to perform face-to-face assessments and support the Medical Director in determining hospice eligibility.

The role involves working in multiple settings, including patients' homes and office facilities, with no after-hours on-call requirements and mileage reimbursement. A Nursing Master's degree and RN hospice experience are required, along with CPR certification and an active state license.

Qualifications

• Must have RN Hospice experience.
• Graduate of an accredited school of nursing.
• Currently licensed to practice in the state serviced.
• Master’s in Nursing.
• Must obtain and maintain CPR Certification.

Responsibilities

• Provides face-to-face assessment to determine hospice eligibility.
• Supports Medical Director and team in care planning.
• Establishes a care plan for continued needs.
• Presents patient assessment information to the supervising physician.
• Completes and submits all required documentation in a timely manner.
