Job summary
CommonSpirit Health at Home is seeking a Nurse Practitioner for PRN hospice coverage in the Houston area. You will perform face-to-face assessments to help determine hospice eligibility and support the Medical Director and team in patient care planning.
The role requires RN licensure with a Master's in Nursing and Hospice experience, CPR certification, and a valid driver’s license. You will work across settings including patient homes and hospice facilities, with emphasis on compassionate,
Qualifications
- Must be a Nurse Practitioner with Hospice experience.
- Graduate of an accredited school of nursing.
- Currently licensed to practice in the state served, with a Master’s in Nursing.
- CPR certification required.
- Valid driver’s license and auto insurance.
- One year of nursing experience preferred.
Responsibilities
- Provides face-to-face assessment services to determine hospice eligibility.
- Supports Medical Director and team in care planning across settings.
- Educates patients and caregivers on care plans and medications.
- Coordinates services with other disciplines and community resources as needed.
- Ensures timely documentation and HIPAA compliance.
- Contributes to development of educational programs in the hospice program.
