Job summary

Bristol Hospice - Hawaii Llc is seeking a PRN Registered Nurse in Honolulu, HI to provide compassionate, one-on-one hospice care. You will develop care plans, assess patient needs, monitor vitals, administer medications, and educate families to support comfort and dignity.

Qualifications

• Must hold a valid Registered Nurse (RN) license in Hawaii.
• Current CPR certification required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Develop and implement individualized care plans based on medical history.
• Deliver one-on-one patient and family support with dignity and comfort.
• Assess physical, emotional, and spiritual needs throughout the care journey.
• Monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and families on care techniques and symptom management.
• Track and communicate changes in patient condition for timely response.
• Maintain detailed clinical documentation of patient progress and quality of care.
• Collaborate with interdisciplinary team to ensure comprehensive care.

Qualifications and Duties

• Develop and implement individualized care plans based on current and prior medical history
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Maintain detailed and accurate clinical documentation reflecting patient progress and quality of care
• Collaborate with interdisciplinary team members to ensure comprehensive care delivery
• Participate in on-call duties, as defined by the on-call policy
• Perform additional duties as assigned in support of patient care and team success
