Qualifications

Responsibilities

• Provide specialized nursing care related to end-of-life and hospice services.
• Collaborate with hospice aides, occupational therapists, physical therapists, social workers, speech therapists, dietitians, and clinical supervisors to support patient care.
• Use, maintain, and prepare equipment and supplies while following appropriate aseptic techniques.
• Provide effective pain and symptom management interventions.
• Monitor and evaluate patient responses to treatments and medications.
• Participate in professional development and continuing education opportunities.
• Support quality assessment and performance improvement initiatives within the organization.
• Perform delegated duties as assigned by the Registered Nurse.
• Complete additional duties and responsibilities as assigned.

Your Responsibilities Include

What You Bring

• Current Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in the applicable state.
• Minimum of three (3) years of nursing experience required.
• Community health, hospice, or medical-surgical nursing experience preferred.
• Understanding of hospice philosophy and the unique needs of terminally ill patients and their families.
• Ability to work flexible hours as needed.
• Commitment to maintaining proficiency with information systems and applicable software programs.
