Job summary

Angels Care Hospice seeks a Nurse Practitioner to visit patients, perform comprehensive assessments, and evaluate hospice eligibility in collaboration with the hospice interdisciplinary team. The role emphasizes compassionate care while following state nurse practice guidelines.

The NP will work under physician supervision, coordinate with the medical director, and participate in plan-of-care development with the interdisciplinary group to ensure high-quality hospice services.

Responsibilities

• Provides medical care and manages terminal illness in compliance with state Nurse Practice Act.
• Completes history and physical examinations by evaluating medical condition and health history.
• Manages health conditions and terminal illnesses.
• Completes hospice face-to-face visit requirements.
• Provides clinical information to hospice medical director regarding eligibility for hospice recertification.
• Makes recommendations to hospice medical director regarding eligibility for certification and recertification.
• Reviews clinical information and coordinates patient care with the interdisciplinary team.
