Job summary

Angels Care Hospice in Nebraska seeks a licensed nurse practitioner to visit patients, perform comprehensive assessments, and evaluate hospice eligibility in collaboration with the interdisciplinary team.

Under physician supervision, you will manage terminal illnesses, communicate with the medical director, educate families, and help develop care plans while ensuring compliance with state regulations.

Qualifications

  • Active NP license in Nebraska or eligible state.
  • Experience in hospice or palliative care is preferred.
  • Strong communication and teamwork skills.

Responsibilities

  • Provide medical care and manage terminal illness in compliance with the state Nurse Practice Act.
  • Complete history and physical examinations by evaluating the patient’s medical condition and health history.
  • Assess hospice eligibility in collaboration with the hospice interdisciplinary team.
  • Provide clinical information to the hospice medical director for interpretation for eligibility for hospice certification.
  • Coordinate and oversee patient care with the hospice interdisciplinary team and other healthcare practitioners.
  • Develop, review, and propose treatment protocols and options for interventions.
  • Educate and counsel patients, families, and caregivers on preventive care, medical issues, psychology, and spiritual needs.
