Job summary

Angels Care Hospice seeks a Nurse Practitioner to provide medical care to hospice patients under physician supervision, perform physical and face-to-face assessments, and evaluate hospice eligibility with the interdisciplinary team.

Qualifications

• NP license or certification
• Experience with hospice or palliative care preferred
• Ability to perform comprehensive history and physical examinations
• Ability to collaborate with interdisciplinary team and physicians

Responsibilities

• Provides medical care and palliation for terminal illness under physician supervision.
• Completes history and physical examinations and evaluates health status.
• Manages health conditions and terminal illnesses in hospice setting.
• Performs hospice face-to-face visit requirements.
• Coordinates with hospice medical director on eligibility and recertification.
• Develops and reviews care plans with the interdisciplinary team.
