• Evaluates hospice eligibility in consultation with the hospice interdisciplinary team.
• Completes hospice face-to-face visit requirements.
• Coordinates and oversees patient care in collaboration with the interdisciplinary team.
• Reviews and develops treatment protocols and proposes options for interventions.
• Educates and counsels patients, families, and caregivers on care plans.
