Job summary

GBMC HealthCare, Gilchrist Towson Center, seeks a Nurse Practitioner to oversee hospice patient care within the inpatient setting. The role includes initiating evaluations, managing complex symptoms, and coordinating with the IDT to ensure quality end-of-life care.

The candidate should have NP licensure in Maryland, with strong hospice experience and the ability to guide staff through clinical protocols; experience with integrative care is a plus.

Qualifications

• Current Maryland NP license or eligibility.
• Experience in hospice/palliative care preferred.
• Ability to lead interdisciplinary teams and educate staff.

Responsibilities

• Perform initial evaluation and treatment of hospice patients.
• Manage symptoms and coordinate care with IDT.
• Conduct family meetings and discuss goals of care.
• Complete MOLST and death certificates as required.
• Attend weekly interdisciplinary team meetings and coordinate discharges.
• Provide after-hours coverage and telephonic follow-up.
