This role acts as a bridge between hospitals and hospice services to improve care for terminally ill patients. Education required includes a Master's in Nursing with GA APRN licensure and NP certification.

Qualifications

• Graduate from an accredited school of nursing with a Master's in Nursing.
• GA license for Advanced Practice Nursing and current NP certification.
• Two years of Medicare patient management experience; home health preferred.

Responsibilities

• Collaborates with a physician in active practice to deliver end-of-life and palliative care.
• Serves as consultant and educator for healthcare professionals and hospice programs.
• Bridges hospitals and hospices to improve care for terminally ill patients.
