Gentiva seeks a Nurse Practitioner for hospice care in Arlington Heights, IL. The role involves comprehensive patient management, collaboration with interdisciplinary teams, and supporting end-of-life care in hospice/palliative settings.

Strong clinical assessment and communication skills are essential. Responsibilities include performing F2F visits, coordinating with treating physicians, and ensuring compliance with Medicare/Medicaid and applicable regulations.

Qualifications

• MSN or higher from an accredited program.
• Completion of an accredited Nurse Practitioner program.
• Minimum of two years NP experience preferred.
• Experience in hospice or palliative care preferred.

Responsibilities

• Perform Medicare-required hospice Face-to-Face visits within CMS timeframes.
• Collaborate with IDG/IDT to deliver patient-centered care.
• Coordinate care with PCPs and specialists to plan care.
• Prescribe medications and manage treatments within state regulations.
• Maintain compliant clinical documentation and billing support.
