Job summary

ViaQuest Hospice seeks a Nurse Practitioner to provide hospice-focused care across the service area. You will conduct face-to-face certification visits and handle re-certifications, with responsibilities for data gathering and physician submissions.

Qualified candidates have NP licensure and a nursing background in hospice; travel is required within the assigned region, supported by comprehensive training and a full benefits package.

Qualifications

• Graduate of an accredited Nurse Practitioner program.
• NP license.
• Hospice experience preferred.
• Valid driver's license, car insurance and reliable transportation.
• Willingness to travel throughout assigned service area.

Responsibilities

• Performing face to face certification and re-certification visits.
• Gathering clinical data for re-certification and submitting to physician or medical director.
• Performing comprehensive physical assessments.
• Performing case management functions and overseeing discharge planning.
• Maintaining current patient records to clearly state problems, plans, actions, goals and changes to patient status.
