Job summary

Parker Jewish Institute Comprehensive Community Hospice in New Hyde Park, NY is seeking a dedicated full-time Nurse Practitioner. You will provide nursing care to terminally ill hospice patients, support families in home care and bereavement, and work with the hospice team to determine initial and continued eligibility.

Qualifications

• Current New York State license as a Nurse Practitioner (NP).
• Degree from an accredited NP program.
• 1+ years of experience as an NP.
• Previous hospice or home care experience a plus.
• Able to use computer technology and electronic medical records in the field.

Responsibilities

• Provide nursing care to terminally ill hospice patients.
• Assist families in home care situations and bereavement.
• Work with the hospice team to determine eligibility for services.
• Coordinate care and ensure comfort and dignity for patients.
