Job summary

Parker Jewish Institute for Health Care and Rehabilitation is seeking a full-time Hospice Social Worker (LMSW) to join the hospice department in New Hyde Park, NY. The role reports to the Hospice Administrator and focuses on delivering comprehensive psychosocial and bereavement support to patients, families, and caregivers.

The position includes field visits in Queens and Nassau counties, with in-patient visits at Parker's facility.

Qualifications

• Master's degree in Social Work required.
• Active NYS Social Work License required.
• Prior experience as a Social Worker in a community health, hospice or inpatient setting.
• Valid New York State driver's license and ability to make field visits in Queens and Nassau counties.

Responsibilities

• Provide social work services to patients, residents, families and significant others.
• Perform psychosocial, emotional, spiritual and bereavement assessments.
• Develop and implement goal-directed IDG care plans and advocate for patients and families.
• Conduct field visits in Queens and Nassau counties and in-patient visits at the facility.
