Job summary

The Parker Jewish Institute for Healthcare and Rehabilitation in New Hyde Park, NY is seeking a Nurse Practitioner to join our Medicine Department on full-time basis. Shifts include 11:00am–11:00pm or 3:00pm–11:00pm, working with medical leadership to deliver compassionate care to residents.

Responsibilities include exam and care planning, resident and family education, documentation, and supervising students while collaborating with a multidisciplinary team.

Qualifications

• Graduate of an accredited nurse practitioner program.
• Active NY NP license with prescriptive privileges.
• NY S Board Certified is required.
• Proficient in SigmaCare or another EMR software.
• Two or more years of experience in a sub-acute or long term care facility; geriatric population preferred.

Responsibilities

• Examining residents and delivering quality care in varied settings including subacute, long term, rehab and outpatient care.
• Assist in resident/patient/family education.
• Re-examine, re-evaluate and modify care plans as necessary, including maintaining proper documentation in resident charts.
• Review all diagnostic testing results and make recommendations to findings.
• Educate and supervisor rotating medical and physician assistant students.

The Parker Jewish Institute for Healthcare and Rehabilitation, located in New Hyde Park, NY, is seeking a Nurse Practitioner to join our team of dedicated specialists working in the Medicine Department.

Now hiring for the following shifts: 11:00am-11:00pm or 3:00pm-11:00pm

In this role working alongside caring, dedicated, and experienced Medical leadership, the Nurse Practitioner will report to the Medical Director, or the supervisory Attending Physician, and is responsible for the delivery of compassionate and quality medical care of assigned residents in coordination with other members of the health care team.
