Job summary

Pennant Group is seeking a Nurse Practitioner to deliver comprehensive hospice services under the supervision of a collaborating physician. This role involves evaluating health histories, diagnosing conditions, and managing hospice care within patients' homes and communities.

You will perform physical assessments, coordinate with the hospice interdisciplinary team, and develop individualized plans of care in compliance with the state Nurse Practice Act and local coverage determinations.

Qualifications

• Nurse Practitioner with advanced graduate education and clinical training.
• Ability to evaluate health histories, diagnose conditions and manage hospice care.
• Experience delivering hospice services under physician supervision.

Responsibilities

• Provides medical care, evaluates and manages terminal illness under supervision.
• Completes history and physical examinations, orders and interprets tests, documents findings.
• Develops individualized plans of care, prescribes medications, coordinates hospice services.
• Performs hospice face-to-face visits and assesses eligibility with the hospice team.
• Advises on patient eligibility for certification/recertification and coordinates care.
• Reviews and oversees patient care with the hospice interdisciplinary team.
• Ensures symptom control through collaboration with team members.
• Reviews and develops treatment protocols and proposes interventions with the physician.
• Consults the physician when plan of care is outside standard practice.
